This form is used to record Health Care Cover details for an overseas applicant. Multiple entries can be made in this form.

The dates entered in this form are used in calculating the Total Duration in the 'Maintain Admission Application Healthcare Cover Letter' (ADMF3910) form and OSHC Amounts in the 'Override Admission Application Healthcare Cover Letter Details' (ADMF3920) form. It is strongly recommended you ensure the dates entered in this form (ADMF32U0) are compatible with the dates derived in ADMF3910 and ADMF3920.

This Form may be accessed from:

  • Directly from the menu.
  • The Direct Admissions Person form (ADMF3000), only when the Health Provider button (middle block) is selected (refer to ADMF2M30).
  • The Record Enrolment form - Person screen (ENRF3000), only when the Health Provider button is selected (refer to ENRF0192).

The Person block contains:

  • Person ID
  • Sex
  • Date of Birth

The Health Care Cover block contains:

  • Start Date
  • End Date
  • Health Care Provider
  • Health Care Type
  • Health Care Number

An entry must exist in this form in order for the OSHC amount to be calculated in ADMF3920.

The Health Care Type is relevant in order for OSHC amounts to be derived. The Health Care Type in this form must be the same as the Health Care Type selected in ADMF3910; otherwise the OSHC amount will return NULL.

Details recorded in this form include:

  • The Start Date and End Date of the Health Care cover
  • The Health Care Provider code
  • The type of Health Care cover
  • The Health Care membership or policy number

Notes:

There must be at least one entry in this form for the applicable Health Care Provider and Health Care Type (selected in ADMF3910), in order for an OSHC amount to be calculated in ADMF3920.

The start and end dates for a Health Care Type, regardless of the provider, should not overlap. If the system detects an overlap, the following warning message is displayed:

'Health Care Cover overlaps an existing Health Care Cover for the same Health Care Type. Continue?'
