This form is used to record Health Care Cover details for an overseas admission applicant.

Directly from menus.

The Direct Admissions Person form (ADMF3000), only when the Health Provider button is enabled (refer ADMF2M30).

The Record Enrolments form - Person screen, only when the Health Provider button is enabled (refer ENRF0192).

Using the Form

Detail recorded in this form includes:

· The start and end dates of the health care cover.

· The code of the health care provider.

· The type of health care cover.

· The health care membership or policy number.
