Purpose

This form is used to set up the list of valid Health Care Provider codes. Health Care Provider codes can be recorded against an international applicant in ADMF32U0 as part of the admission process for international students.

Notes/Rules

· Once used in the System, records cannot be deleted. Select a record's Closed indicator to stop any future use of that Health Care Provider code.
