The purpose of this form is to enquire on an applicant's responses to questions that were asked during their application in Applicant Portal, and if requested by the applicant, edit their text responses. Administrators can sort the response records by Start date or Service Code. This form is accessed via the Questions button in ADMF5000 in context of an applicant. |

Rules/Notes: The Applicant Service Block There are three Response Types: Multi-choice, Yes/No, and Open. When the selected Service has a YES-NO Response Type, a YES or NO response is displayed in the Response field. In this case the Open Question Response field and the Multiple Choice Response block are disabled. When the selected Service has an OPEN Response Type, a response is shown in the Open Question Response field. When the selected Service has a MULTIPLE CHOICE Response Type, all text answers are displayed in the Multiple Choice Response block and at least one answer must be selected. Multi-choice open text responses are displayed in the Multiple Choice Response block. The Multiple Choice Response block is only enabled if the selected Service has a Response Type of MULTI CHOICE. When you select the Question Text button, the Question Text field displays in place of the Open Question Response field. The Question Text field displays the question for which a response, shown in the Open Question Response field, was given. The Sort By field is used to sort records by Start Date (desc) and Service Code (asc), or Service Code (asc) and Start Date (desc). The default sort order is by Start Date (desc) and Service Code (asc). The As At Date field: The As At Date field is used to restrict the display of Service records to those that are current as at the specified date (i.e. where the As At Date falls within the Start and End Date range for the service record).
